step 1 - Create account for background check

Go to cbc.az.gov 


Note- If you made an account when you applied for your fingerprint card, that log in works on this site. If not, follow the steps in the video to create your account.


Choose "Individual/Personal" account


Remember to check your email to verify your new account!

Step 2 - Submit your background check

Log in to your account at cbc.az.gov


Select "Employment" background check


Employer's email: 1099@abrighteravenue.org


Fingerprint card required?: Yes


Note - If you have previous expired fingerprint cards, the system currently glitches after you click Yes. If this happens, email CBCtechnicalSupport@azdes.gov with the subject "FCC looping issue" from the email address associated with your account. Give your name and fingerprint card number.
