We are always looking to maximize our providers' schedules! We may have a client in your area waiting for services that you would be the perfect provider for. If not, we will search the DDD database for an available client needing a provider like yourself. Please use this form to let us know of your availability: HOURS REQUEST FORM

Second, you can view the auto-visit video a the Learning Center. Log in a www.spokechoice.com and click Learning Center. Then choose the video in English or Spanish.

$19.00 per hour - Respite
$20.00 per hour - Attendant Care
$22.00 per hour - Habilitation
Pay occurs semi-monthly.
Hours for the first half of the month (the 1st - 15th) are paid on the 30th/31st.
Hours for the second half of the month (the 16th - 30th/31st) are paid on the 15th of the following month.
For example:
January 1st - 15th work is paid on January 31st.
January 16th - 31st work is paid on February 15th.
If pay day falls on a weekend or holiday, payment is made early!
Dependability and a good attitude! To be certified to provide Respite, you need a complete file with all the forms and basic certifications listed at www.abrighteravenue.com/1099. You'll then provide Respite (caring for the client while the main caregiver takes a break) following the schedule you set with the guardian. You'll bill in SpokeChoice as instructed during your orientation.

To provide Attendant Care, you will need all the basic certifications listed at www.abrighteravenue.com/1099 plus the Direct Care Worker Training listed in the green section toward the bottom. This additional class will certify you to provide personal care services to your client. If this wasn't part of your original application process, you can certainly add it later by following the instructions at www.abrighteravenue.com/1099 to complete the class and sending in your certificate when finished.

To provide Habilitation, you will need all the basic certifications listed at www.abrighteravenue.com/1099 plus the Habilitation 101 listed in the green section toward the bottom. This additional class will certify you to work on goals with your client. These might be communication goals, self-help skills, attention to task, or whatever goals your client has. If this wasn't part of your original application process, you can add it later by following the instructions at www.abrighteravenue.com/1099 to complete the class and sending in your certificate when finished.

If you have hours that were due for payment but didn't go through, there could be a few reasons. The hours may not have been guardian approved in time. If you provided only Respite, this is usually the case. If you provided Habilitation or Attendant Care, the data may be missing. You can log in at www.spokechoice.com to see if your hours are management approved. If not, check for one of the missing items above. In either event, if the hours were not guardian approved in time or notes were missing, the hours will go through for payment on the next pay day once the issue is resolved. You can choose to enter your email or phone number (the one we have on file with you) or change the button to "Your Information" where you can enter your name, birthdate, and SSN. Either option you choose should help the system locate you.
It may ask if your employer already uses ADP. Select "Yes".
Once it finds you in the system and pulls up your account with us, it should allow you to create your own username and password to easily log in in the future. From there, you should be able to access your tax forms, as well as your direct deposit account and personal info for updates.
Article 9 is good for 3 years.
CPR/First Aid is good for 2 years.
Fingerprint cards are good for 6 years.
The Criminal History Self Disclosure form needs redone every 3 years.
If you choose to be a transporting provider, we need a copy of your vehicle insurance and registration each time it is renewed.
All contractors and employees with A Brighter Avenue who provide direct service to clients are required to take and maintain current Article 9 training. There are are multiple places that provide this training.

Arizona Provider Training offers this class both in person and online. Go to:
Chose either Classroom Course, choose location, choose language, and choose CPR and First Aid (taken separately in person)

Choose Online-Self Paced, choose language, choose CPR, AED, First Aid blended
Note that if you take this course online, you must take the in person test

You will receive a CPR and First Aid card upon completion. Please provide us with a copy of the front and back of your card in person or by emailing it to 1099@abrighteravenue.org
All contractors and employees of A Brighter Avenue must supply a copy of a valid Level One, Non-IVP Fingerprint Clearance Card - both front and back - within 30 days of hire to avoid suspension. Fingerprints must be digital to register directly with DPS (Department of Public Safety).

Please note: When your fingerprints are taken, you will be given a receipt or a verification. Please supply us with a copy of this receipt/verification as it has your application ID number on it. We need record of this application ID until your fingerprint card arrives in the mail.
When you receive your fingerprint card, please provide us with a copy of the front and back. You can do this in person or by emailing a copy to 1099@abrighteravenue.org
